Notice of Data Security Incident
Monaco & Associates Incorporated (“Monaco & Associates”) is notifying individuals of a data security incident that may have impacted their personal information. We take the privacy and security of all information seriously and sincerely regret any concern or inconvenience this incident may cause.
What Happened?
On or around April 10, 2026, we experienced a cybersecurity incident that limited access to certain computer systems. We immediately initiated our incident response protocols, took steps to secure and contain the incident, engaged cybersecurity experts to assist with an investigation, and notified law enforcement. During this time, we remained operational and continued to provide our services to individuals as regularly scheduled.
On May 01, 2026, our investigation determined that some files belonging to a limited number of individuals may have been accessed by an unauthorized individual. On May 28, 2026, we determined that these files contained the individual’s name and the individual’s name, address, Social Security number, Medicaid ID Number and, in some cases, diagnostic and medication information. While we do not have any evidence that this information has been misused, we are informing individuals about this incident out of an abundance of caution.
What is Monaco & Associates doing?
We want to assure you that we are taking steps to minimize the risk of this happening again in the future. Since the incident, we have worked to restore our systems, tightened our access controls, and are evaluating additional technical safeguards to strengthen the security of our systems.
We are also mailing letters to individuals whose information was identified as potentially impacted over the next few weeks. These letters will contain more information about the incident, complimentary credit monitoring and identity theft protection services, and steps individuals can take to protect themselves.
What You Can Do:
We encourage you to remain vigilant against incidents of identity theft by reviewing bank accounts and other financial statements. Individuals should review credit reports, bank account, and other financial statements, and immediately contact their financial institution if they identify suspicious activity. You can also visit the Federal Trade Commission’s website for more information on protecting your identity at consumer.ftc.gov/identity-theft-and-online-security.
